Halton Tier 2 Weight Management Service

The Halton ‘Fresh Start’ service supports individuals to increase their physical activity levels and control their nutritional intake. This program is available either at face-to-face classes or digitally via the Fresh Start App. Please use the referral form to refer to any of the health improvement services.

There is also a digital leaflet with a QR code and invite ID that any patients can scan with smart phones and sign up to the online version whilst they are in the pharmacy with you. The Halton Health and Welling service have offered to print and send these leaflets to pharmacies, so please look out for them arriving with you soon.

Knowsley Tier 2 Weight Management:

Healthy Knowsley Service supports Knowsley residents(18+) or those with a Knowsley GP to access Lifestyle support, which includes:

Health Coaches – 1:1 support for healthy eating or weight management (Tier2).

The weight management pathway has a number of support options, which include:

• NHS weight management App
• Slimming World (there is criteria for this re previous membership)
• Measure Up –  programme which incorporates weight management 1:1 support and physical activity (criteria for this pathway)
• Registered Dietitians – 1:1 support on a targeted or specialist pathway, which includes bariatric pathway, (included on this pathway CBT).

The service is available 8am-8pm service Monday-Thursday & 8am-5pm Friday.

The service is also the single point of access into Lifestyle services across Knowsley, including stop smoking services and local gyms (Volair) for exercise on referral and also signposting into other support services.
